With technology in general, not everything is easily set up, though I believe its understandable to figure better technology equals less problems, but that isn't always the case. I understand that previously, you never had issues playing the game, but having moved or changing internet plans may have affected your ability to connect. If you moved to a location farther from a server, it may increase your Ping or Latency times. Changing plans, may require you to manually set up your ports or power cycle your rerouter and console to reassign your console IP address.
You can also set up a Static IP address to increase the stability of your connection and it may prevent you from having to set up your ports in the future. In the link that FlavaSprinkles provided, you can set up your ports with UPnp (Universal Plug and Play), Port Forwarding (Manual Set up), or DMZ (Open All). You should only use one of the methods and not the three in conjunction as they are all methods to set ups your connection ports.
If you choose to manually set up your ports, here go the port numbers that you will need:
PS4:
80, 443, 465, 983, 5223, 10070-10080 TCP
3478-3479, 3658, 10070 UDP
Call of Duty Ghosts:
80, 81,443, 3074, 5223 TCP
36081, 3074, 3075, 3478 UDP
If you have issues, setting up your ports or feel uncomfortable with the process, you can feel free to give your ISP a call and they should be able to guide you through the process or may even do it for you.
